Requesting a Medical Leave

Ready to request a medical leave? 

  1. Please review the Medical Leave and Return Process Overview and frequently asked questions about medical leave. 
  2. Complete the Treatment Provider Assessment. The document consists of two parts. Part one should be completed by the student and part two must be completed by the treatment provider. 
  3. The treatment provider must return the assessment directly to the student. Documentation should not be sent directly to OSSA.
  4. Upload the completed Treatment Provider Assessment into the Requesting a Medical Leave form.
  5. Check your email for receipt of your request and guidance on next steps. Most requests not requiring additional information, recieve a decision within 10 business days. 

Current Term

Medical leaves designated as current term are those in which the withdrawal was initiated between the Last Day to Drop with Refund and the Class Ends Date. To find the corresponding dates for these milestones, please visit the academic calendar.

Retroactive Request 

Medical leaves designated as retroactive are those in which the withdrawal was initiated after the Class Ends Date. To find the corresponding dates for these milestones, please visit the academic calendar. Retroactive requests can be made up to the commencement date of the semester one calendar year later.
