This form is to be used for academic complaints (for this purpose, complaints and grievance terminology is used interchangeably) in compliance of Article 4 within the Student Rights and Responsibilities. 

Prior to submitting this form, students are required to attempt informal resolution options with their instructor and the instructor's supervisor (you will be asked to demonstrate your good faith efforts to informally resolve the complaint). The informal grievance process is an opportunity for both the student and the faculty to add clarity surrounding issues that may arise. Students are encouraged to connect with the Ombudsperson's Office and Student Rights Advocates for further support throughout informal and formal resolution channels.
